Standard Consent
Overview
DocNow provides a consent form module for creating text based forms for which a single signature can be requested. The result of this operation is a PDF containing the patient/provider information, form text and patient's signature.
Administrative Portal
Before using this feature the Enable Consent Form practice permission must first be enabled by a DocNow team member. Once the practice permission has been enabled, a consent form can be configured using an Administrative user account and navigating to the Admin Portal>>Dashboard>>Consent screen.
The consent screen displays the following information:
- Sr: Consent form serial number
- Title: The title configured for the consent form. This will be displayed to the provider as a in the consent form selection drop down in a future screen.
- Description: The content of the consent form
- Specialty: The specialty the consent form is assigned to.
- Actions: View/Edit, Delete
Adding a New Consent Form Configuration
- Select the Add New Form button to create a new consent form.
- The consent form dialog provides the following fields:
- Title: The title configured for the consent form. This will be displayed to the provider as a in the consent form selection drop down in a future screen.
- Select Specialty: This dropdown is populated by the specialties configured for your company. Selecting a specialty will display the specialty in the Admin Consent screen
- Consent Expiration: The Consent Signed icon on the patient census will change from Complete to Incomplete for a patient following the number of expiration days. A value of 0 indicates that the signed form will never expire. A value of 1 represents that the consent for is valid for the current DOS only and will reset on the subsequent DOS if the patient happens to be seen 2 days in a row.
- Description (Text Editor): This is the area where the consent form details should be added. This field supports font format, font color, font sizing, highlighting and alignment at this time.
- Signatures: Checkboxes configure support for multiple signatures. All signatures are considered optional and will be replaced with an "N/A" if no signature is provided in the generated consent PDF document.
- Select the Save button to save the consent form configuration or Cancel to abandon all changes
Provider Portal
Signing a Consent Form
- A consent form can be signed and associated to a patient by clicking the Consent icon from the Daily Census screen.
- A consent icon without a checkmark means a consent form has not been signed for this patient on this Date of Service or that a previous consent form has expired allowing an additional opportunity to sign a new consent form.
- A consent icon with a checkmark means that a consent form has been signed.
- Consent status (signed/unsigned) will reset based on the expiration configured for the form (See Admin section above)
- When the Consent icon is clicked the Consent Form dialog is displayed. Providers can select the appropriate consent form by using the dropdown at the top of the screen.
- The signer should use the drawing area to add their signature.
- Select Clear Signature to clear the draft signature and try again
- Click the Next Signature button to navigate to the next signature or use the tabs (e.g. Patient Signature, POA Signature, Provider Signature, etc.)
- Once the final signature page is reached the user can select Complete Signatures to complete the form and generate a PDF document.
Viewing the Signed Consent Form
- Once signed the consent form PDF can be view in two areas within the DocNow software
- View the consent form by selecting the consent form icon from the Daily Census from which the form was signed
- Navigate to the Patient Profile>>Patient Documents screen to view or download the PDF.
