Introduction
A QAPI report in wound care refers to a Quality Assurance and Performance Improvement report that tracks and evaluates the quality of wound care provided within a healthcare setting. This report includes data on wound healing trends, amputation cases, and facility-acquired wounds.

QAPI Report for Wounds
Available Filters:
- Select Tab
- Other - Reports on all Active and In Remission
- Amputations - Report on all wounds marked as Amputation
- Facility - The facility to report on (Single select dropdown)
- Date Range - Filters results based on the selected date range
- Healing Status
- Deteriorating - Wound Area Change % has increased since the initial assessment
- Healed - Wound Area Change % or Wound is in Resolved status or Wound is in Remission
- Improving - Wound Area Change % has decreased since the initial assessment
- Open/Initial Only - The wound only has an initial assessment therefore Improving/Deteriorating cannot be evaluated at this time
- Stalled - Wound Area Change % has no change from the initial assessment
- Report Current Wound Status
- Unchecked - The wound Healing Status, Location, Status, Etiology and Severity are reported based on the last assessment found within the filtered date range
- Checked/Enabled - The wound Healing Status, Location, Status, Etiology and Severity are reported based on the most recent assessment for the wound even if this is outside the filtered date range.

Wounds are categorized based on Area Change percentage:
- Improving: Area Change < 0%
- Stalled: Area Change = 0%
- Deteriorating: Area Change > 0%
- In Remission: Area Change = 100%
Frequently Asked Questions
- Are facility-acquired wounds identified automatically?
- Yes, wounds are classified as facility-acquired if they have the wound_recognition_type set accordingly.
- Why doesn’t the report calculate a rehospitalization rate?
- The report only displays patients who were readmitted due to wound complications. It does not perform rate calculations.
