Wound association: Wounds are always linked to a census.
- Phrase resolution: The .woundassessment_all and .woundplan_all phrases only resolve for unlocked wounds and their revisions.
- Locked wounds: When a note is signed, wounds for that date of service (DOS) become locked, and the .woundassessment and .woundplan phrases will no longer resolve.
- Census integrity: Do not create a census on a past date and later update a future census date to match the past date. This will not ensure proper wound flow and phrase resolution.
