• Understanding how wounds are associated with census records, how notes impact wound status, and how to properly revise locked wounds ensures that phrase resolution functions correctly within the system.

Key Points

• Wound association: Wounds are always linked to a census.

• Phrase resolution: The .woundassessment_all and .woundplan_all phrases only resolve for unlocked wounds and their revisions.
• Locked wounds: When a note is signed, wounds for that date of service (DOS) become locked, and the .woundassessment and .woundplan phrases will no longer resolve.
• Census integrity: Do not create a census on a past date and later update a future census date to match the past date. This will not ensure proper wound flow and phrase resolution.

Notes on Correct Workflow

A) Creating a new wound or revising an existing wound:

• 1. Start by creating a new census.

  2. Create a note within that census.

  3. Add the wound within the same census.

B) Ensuring proper workflow:

• • Always create the census correctly and associate wounds with the appropriate census.

  • Avoid modifying census dates to bypass system logic, as this will disrupt wound phrase resolution.
