Purpose
- The Wound Assessment Report allows providers to review, download, or upload comprehensive data for individual or grouped wounds across a selected date of service (DOS). This tool supports clinical decision-making and documentation by consolidating patient wound data in a structured and printable format.

Considerations
- Patient Search: To begin, the provider must enter the patient’s name in the search field. The system will automatically populate the Date of Birth (DOB) field based on the match.

Facility and Provider filters are optional and can help narrow search results.

A field labeled Notes DOS will appear after patient selection. This allows users to choose from a list of documented Dates of Service.

Grouped Wound Assessment Reports (summarizing all wounds on a patient)

Instructions / How to

Search for a Patient.

Begin by typing the patient’s full name (minimum 4 characters).

Once selected, the DOB will automatically populate.

Apply Filters (Optional)

- Use the Facility and Provider filters to refine the report scope if needed.

Select Date of Service

- Use the Notes DOS dropdown to select the specific Date of Service. This controls which wound entries will be displayed.

Viewing the Report

- The report will display key clinical details, including:

- Wound Number, Location, Etiology, Stage, Status, Date Acquired

Measurements (Length, Width, Depth, Area, Volume)
- Observations such as:

- % Epithelial / Granulation / Slough / Eschar
- Tunneling, Undermining, Periwound Condition

- Wound status highlights like days active, area, and volume changes are clearly visualized for quick assessment.
- The report will display key clinical details, including:

Uploading or Downloading Reports

- Use the icons above the report (⬆ Upload, ⬇ Download, 👁 View) to manage the report files.
- You can upload or download Grouped or Single Wound Assessment Reports depending on your selection.

Additional Options

- Add Skin Sub Referral: Initiate a referral to another provider directly from the report screen.
- Move Wound: Reassign a wound to a different census or patient if necessary.

Troubleshooting
- No Patient Found: Ensure the full name is entered correctly (minimum 4 characters).
- Notes DOS Not Appearing: Confirm a valid patient has been selected and that wound notes exist for the date range.
- Upload Fails: Verify that the file format and size meet system requirements.
- Incorrect Wound Assignment: Use the Move Wound option to correct patient or census assignment.
