Wound Assessment Dashboard

Purpose

• The Wound Assessment Dashboard displays a comprehensive list of patients with active wounds at a selected facility. This tool allows providers to review wound activity during a defined time range, transfer patients to a census, and access supporting notes and documentation.

Considerations

• Precondition:
  • • The user must first select a Facility in order to view the dashboard content.
• Patient List Scope:
  • • Once a facility is selected, the system will automatically display patients with wounds created during the selected timeframe (e.g., Jun 26, 2024 – Jul 26, 2024).
• Tabs Available:
  • • Wounds (default view)
    • Patient Notes
    • Patient Documents

Instructions / How to

1. Select a Facility
   1. • Use the Facility dropdown at the top left to choose the facility you'd like to view. This is required to populate the patient list.
2. Viewing the Patient List
   1. • Once a facility is selected, the dashboard will display a list of patients with wound entries that fall within the selected date range. Each patient entry shows:
        • • Name and Date of Birth
          • Facility and Medical Record Number (MRN)
          • Last Seen Date
          • Wound Summary (e.g., wound name, type, stage, duration, % area change, % volume change)
3. Viewing wound Details
   1. • Wounds: Lists all wound entries with measurement history
      • Patient Notes: Clinical notes associated with the patient
      • Patient Documents: Attached PDFs or files relevant to the patient
4. Transferring Patients to a Census
   1. • Use the checkboxes to select one or more patients from the list.
      • Click "Add to Census" to move the selected patients to a different census within the selected facility.
5. Download & Refresh Options
   1. • Use Download Wound Images to export wound photos (if available).
      • Click Refresh Wounds to reload data and ensure the most current records are displayed.

Additional Notes

Only active wounds are displayed in the default view. Users can toggle between Active, In Remission, Amputated, or Archived tabs as needed.

Troubleshooting

• No Patients Displayed:
  • • Make sure a facility is selected and the date range includes wound entries. Try expanding the timeframe if needed.
• Wound Details Not Expanding:7
  • • Ensure you’re clicking the arrow next to the patient’s name. If nothing opens, try refreshing the browser.
• Unable to Add to Census:
  • • Verify that the selected patients are eligible and that the census functionality is enabled for your account.
