- Table Definitions

| Facility | Name of the medical facility associated with the patient. |
| Entity | The medical company or sub-practice |
| Patient | Full name of the patient. |
| Patient ID | Unique system-assigned patient identifier. |
| Facility ID | Unique ID for the facility where the patient is/was admitted. |
| Admit Date | Date when the patient was admitted. |
| Admit Time | Timestamp of admission. |
| Admitted By | Provider or staff member who entered the admission. |
| Patient Status | Whether the patient is currently Admitted or Discharged. |
| Encounters | Number of documented encounters since admission. |
| Last Seen Date of Service | Date of the patient’s most recent encounter. |
| Last Seen Provider | Name of the provider who conducted the most recent visit. |
| Days Since Last Seen | Count of days since the patient was last seen. |
| p.source | Refers to the data origin, such as PointClickCare, or N/A if not applicable. |
| Readmission | Displays YES or NO to indicate if the patient was readmitted within 30 days. |
| Qualify By | Indicates if the patient was “Seen within 30 days” or “Not seen in last 30 days.” |
| Active/Dormant/Inactive | Status classification based on encounter activity: • Active: recently seen • Dormant: no recent encounters • Inactive: long-term lapse in engagement |

- Pagination Controls: Choose the number of records to display (e.g., 200 entries per page).
- Column Sorting: Click on any column header to sort by date, provider, facility, etc.
- Export Options: From the three-dot menu (⋮) in the upper right:
- Export to .CSV or .Excel (current view only)
- Export full report as .CSV or .Excel
- Download as image for documentation or presentation
- Force Refresh: Instantly refresh report data to reflect the latest system updates.
- Fullscreen Mode: Maximize the report panel for easier navigation.

Troubleshooting
| Issue | Resolution |
|---|---|
| Missing patient records | Ensure the patient has been admitted within your facility and is not filtered out by entry limits. |
| Export file incomplete | Use “Export to full .CSV” or “Export to full Excel” to ensure all rows are included. |
| Stale data | Use “Force refresh” from the top-right menu to pull the latest data. |
| Unclear readmission status | Confirm if the patient was re-admitted within 30 days; otherwise the system will flag as “NO.” |
| "p.source" shows N/A | This field is blank when no external source integration (e.g. PointClickCare) is associated. |
