Purpose

• This article guides users through the process of discharging patients from the active census, managing patient discharge history, and understanding demographic reporting within DocNow.

Considerations

• A patient with signed or uploaded notes may trigger system alerts preventing immediate discharge to ensure documentation completeness.
• The Daily Census Report reflects only currently active patients. Discharged patients will not appear on this report.
• Historical discharge information should be viewed via the 'Admit/Discharge History' tab.

Description & Features

DocNow provides clear workflows for managing patient discharge and demographic records:

• Discharge Actions: Initiate patient discharge directly from the census with a reason for discharge.
• Patient Readmission: Easily re-admit previously discharged patients using the 'Search and Add Patients' function.
• Admission & Discharge History: Provides a detailed log within patient records, tracking admissions, discharges, dates, reasons, facilities, and responsible providers.
• Daily Census Report: Offers a downloadable report exclusively for active patient tracking, with planned discharge dates clearly marked.
• Record Management: The system includes functionality for deleting patient discharge records after specified periods, distinctly separate from viewing historical data.

Steps

Discharging a Patient:

1. Locate the patient entry in your active census.
2. Click the 'Delete' icon under the 'Actions' column.
3. In the dialog box, select a discharge reason (e.g., 'Discharged home').
4. Click 'Yes' to confirm the discharge.

Viewing Discharge History:

1. Access the patient's individual record by clicking their name.
2. Navigate to the 'Admit/Discharge History' tab on the left-hand menu.
3. Review the detailed admission and discharge events log.

Troubleshooting

1. Cannot Discharge Patient: Verify if there are pending signed or uploaded notes; address these alerts to proceed.
2. Missing Discharged Patients in Reports: Remember that discharged patients do not appear on the active census. Always check the 'Admit/Discharge History' tab for historical data.
