Create Notes, Open Charts, Document Wounds, and Capture Charges
From any patient row in the census, open notes, patient charts, wound documentation, and billing without navigating away from your round. Documentation and billing status is visible on every row so you always know exactly where each patient stands.
Clinical actions from Census
5Create a note
Click the note button on any patient row to open the note template picker and start a new note without leaving the census. The template picker shows the note types available for that patient's encounter at your facility. The button appears only when you have the role and facility membership required to document for that patient.
Open the patient chart
A chart button on each row opens the full patient record for review or more detailed data entry. Closing the chart returns you directly to the census — not the dashboard. Use the breadcrumb at the top of the chart to jump back to your round at any time.
Access wound documentation
For patients with active wound assessments, a wound button on the row opens wound management directly. Skip navigating through the patient chart to reach wound records — the button takes you there in one tap.
Capture charges
A billing button on each row opens charge capture for that patient's current facility visit. Use the Documentation Status filter set to Not Submitted to surface patients with outstanding billing and work through them row by row.
Documentation status at a glance
Every patient row shows the current state of their Note, Wound, and Billing items using the action buttons themselves — a blank button means nothing has been started, a draft indicator means work is in progress, and a check or submitted badge means the item is signed or submitted. You know exactly where each patient stands without opening a single chart.

Comments and collaboration
5Add a comment
Each patient row has a comment thread. Click the comment button to open the thread and leave context for teammates — visit notes, follow-up reminders, or family information. Comments stay attached to that patient in the census and are visible to all team members with access.

A chart button on each row opens the full patient record — close the chart and you return directly to the census.
Reply to a comment
Reply directly to any comment to keep related context threaded together. Replies stay connected to the original comment so conversations remain readable over time.

For patients with active wound assessments, the wound button opens wound management directly — no chart navigation required.
Mention a colleague
Type @ in any comment to open the mention picker and select a colleague by name. The mentioned provider receives a notification with a direct link back to that patient in the census — no copying, no searching.

The billing button on each row opens charge capture for that patient's current facility visit — combine with the Not Submitted filter to close out billing fast.
Edit or delete a comment
Authors can correct or remove their own comments. Deleted comments leave a visible placeholder so the thread structure stays readable and teammates can see that something was there.

Mentioned providers receive a notification badge in the notification center. Opening it shows the mention in context with a direct link to the patient in the census.
