Clinical Notes
8Combined notes list
Draft and signed notes are now shown together in a single list — no more switching between tabs. Drafts surface at the top, and a "Drafts only" quick filter lets you narrow the view.
Download a note as PDF
An Export menu in the note header lets you download any note as a PDF. Signed notes include the full e-signature block; drafts are watermarked "DRAFT — not signed."
Strike out a signed note
Providers can now strike out a signed note to withdraw it from the active record without deleting it. Struck-out notes remain visible in the list with a badge showing who struck them, when, and why — preserving the full audit trail.
Required-field navigation buttons
The note editor toolbar now shows Prev and Next buttons with a live count of unfilled required fields, so you can step through them by mouse without needing keyboard shortcuts.

Template editor: Save & Close and in-place Save
When editing a note template, you can now save changes without leaving the editor, or Save & Close to return to the template list in one step.
Template toolbar tooltips and named ad-hoc dropdowns
Every toolbar button in the template editor now shows a helpful tooltip on hover. Ad-hoc dropdowns in a template can be given a name so providers see "Choose: Wound Location" instead of the generic "Choose…".

Template section management for admins
Customer Admins can now choose which note sections their practice uses and rename their labels — without changing the underlying clinical identity of each section. The catalog has expanded from 17 to 25 entries.
Duplicate note warning
Creating a note when one already exists for the same patient and date now shows a confirmation dialog, preventing accidental duplicates.
Patient Census
6Export census notes
You can now export all signed notes for a census as a single combined PDF, or as a per-patient ZIP archive. Export is available from the census list and from the individual census page.
Rename your census
Censuses can now be named and renamed after creation. The census list now shows a dedicated Census Name column alongside Facility.
Multiple censuses per day
Creating a second census for the same facility and day is now allowed. DocNow warns you with a confirmation and suggests a name with a -2 suffix that you can edit.
Census list loads faster
The Censuses list now uses server-side pagination so it stays responsive as your history grows. Filters for date range, location, status, and name are applied server-side as well.

Copy-forward shows only recent censuses
The copy-forward selector when creating a new census on Mobile Care now shows only active and recent censuses — deleted or old rounds no longer clutter the list.
Census tab in the navigation bar
A persistent tab in the top nav remembers your last-opened census or facility and returns you there in one click — so you never lose your place while moving between patient charts.

7Search by name, MRN, or date of birth
The patient list search now matches against name, MRN, and DOB — useful when you have a wristband or referral slip in hand rather than just a name.
Flags column with inline add/remove
The patient list now shows a dedicated Flags column. You can add or remove a flag directly from the list without opening the chart. The term "Alerts" has been renamed to "Flags" throughout the app.
Admit Date and Last Seen columns
Two new columns show when a patient was admitted and when they were last seen in a session encounter, giving rounding context at a glance.
Add patients manually
You can now create a patient directly in DocNow without requiring an EHR import. The Add Patient button opens a quick-add form (name + DOB + optional MRN) with expandable demographics.

Customizable patient list settings
A gear icon on the patient list toolbar opens a settings modal — control default row-click action, default sort, column visibility, and print preferences, persisted per user.

"Add to Census" now opens a modal that defaults to today's census, lets you pick a future census, or create one from the selected patients on the spot.

The Note, Wound, and Order action buttons on each patient row now show whether documentation already exists for today. Clicking opens the existing document rather than always creating a new one.

Encounters
2Assign participants to an encounter
Providers can now add participating practitioners to a session encounter — from the CareTeam or by ad-hoc search — with roles such as Consulting, Supervising, or Primary. The creating provider is stamped as the attending automatically.

Void an empty encounter
A session encounter with no signed notes can now be voided from the Outpatient Encounters tab, closing it cleanly rather than leaving it open indefinitely.

Wound Care
3Capture wound photos with your device camera
When adding wound photos, you can now tap "Take Photo" to use your device's camera directly — no need to capture elsewhere and re-upload. Photos are sanitized (EXIF/GPS stripped) and optimized automatically.
Streamlined wound entry
Clicking "+ Add Wound" now takes you straight into the initial assessment workspace. Nothing is saved until you confirm, so abandoned flows no longer leave empty wounds behind. Row actions for existing wounds reflect the visit state: "Follow-Up Assessment", "Edit Existing Assessment", or read-only after signing.
Encounter-aware wound status
The wound list now shows whether a wound was assessed in the current visit specifically, not just historically. Carry-forward badges have been removed from assessment fields for a cleaner view. A lock indicator explains why wounds can't be added when the note is already signed.
Notifications
2Explicit notification read control
Notifications now stay unread until you deliberately mark them — opening the notification bell no longer changes read state. Use the per-item Read control or "Mark all as read" to acknowledge notifications.

Improved notification popup
The notification dropdown now shows a scannable Patient · Sender · Message table with up to 8 rows. Clicking a row navigates directly to that patient's Communication section.
Dashboard
2Today's Activity by census
The dashboard now shows one row per census for today — facility name, patient count, and seen vs. not-yet-seen — each linking to that census or its facility view. The previous aggregate band has been removed.
Incomplete encounter details
Each row in the Incomplete Encounters widget now shows the place of service and census name in plain terms, with a clickable row that takes you to the unfinished encounter. A Today filter lets you scope the view to just today's open work.
Mobile Care
1Facility census list and patient finder
The Facility screen now shows a Censuses list and a Find Patients finder that covers all active residents — including residents not yet on any census. Practice-type presets (e.g. Physiatry/PM&R, Cardiology) and quick-filter chips (Newly admitted, Due for a visit, On hospice) help you zero in on the right patients quickly.

1Document type configuration
Customer Admins can now enable or disable any of 21 curated document types for their organization from Organization Settings → Documents. The upload picker reflects the enabled set for all providers immediately.
Patient Chart
4Expanded Problem List editing
The Edit Problem sheet now lets clinicians update Provider and Onset date — not just status. The Problem List now shows Onset and Date Added as distinct columns so clinical and administrative dates are no longer mixed.
View all banner alerts without leaving the page
Overflow Allergies and Flags in the patient banner now expand in place via a popover when you hover or click "+N more" — no more navigating away just to see a hidden item. Navigation is still available by clicking the label or an individual chip.

Consistent search in Imaging and Wounds
Search fields on the Imaging and Wounds chart sections are now always visible, matching the standard used across the rest of the chart.
DocSense dictation control in the patient header
For practices with DocSense enabled, a mic control now appears in the patient header — between the back button and the visit picker — to start and stop dictation and show session status from anywhere in the chart.

Bug Fixes
4Census settings changes now apply immediately
Column visibility and other preferences saved in the census settings modal were sometimes reverting on navigation — this is fixed.
Dot-phrase trigger validation
Creating a dot phrase with an invalid trigger (e.g. one containing spaces) now shows a clear inline field-level error instead of a confusing server error. The sheet stays open so you can correct it immediately.
Add Document dropzone fully clickable
Clicking anywhere in the Add Document upload zone now opens the file picker — previously only the word "browse" was active.
Patient list and banner icon fixes
The calendar view toggle no longer appears on lists where it doesn't apply. The banner expand chevron has moved to the status/location/attending context strip for a cleaner layout.
