Important Updates
Draft Progress Note Auditing Bug Fix
A cloud infrastructure configuration change caused the system to stop saving the raw text field object within the intermediate draft edit history for clinical notes. During this period, the incremental save versions recorded while clinicians were drafting notes were not captured.
Impact
Signed clinical notes, signed addendums, and finalized documentation were not affected.
All final signed records remained intact, accessible, and compliant with clinical documentation requirements.
User access tracking and final document storage continued functioning normally.
The lost data is limited to raw note text field in the version history of save data.
Draft edit history for this period (July 21, 2025 - March 4, 2026) cannot be recovered.
Resolution
The issue was resolved on March 6, 2026.
Draft audit history capture for progress notes is now functioning normally.
DocNow Preventive Actions
DocNow has now added monitoring to detect audit log capture disruptions and has initiated a review of encryption key management processes to prevent recurrence.
New Feature Details
Co-signer Upload Behavior
Company Admins can now adjust the upload behavior of Co-signed progress notes. Previously notes were only uploaded to integrated facilities (i.e. PointClickCare, MatrixCare) upon both the author and co-signer signing the note. A company setting now exists for changing the default behavior.
Admin Portal
Adjust the Co-signing settings by navigating to Admin Portal>>Welcome Banner>>Settings>>Co-sign Settings
Two options exist which are applied to all users with the Co-signer permission enabled:
Option 1: Upload after Co-sign
The note is signed and uploaded only after the co-signer completes the co-signature. This functionality has always been the default behavior in DocNow.
Option 2: Upload at time of initial signature (New functionality)
The note is signed and uploaded immediately when the author signs it (with a co-signer selected), allowing the co-signer to only create an addendum and sign it. When the co-signer opens the note, "Create Addendum" button becomes available.
Unable to Determine Depth
Wound Care users will now have the option to choose Unable to Determine for depth measurements when documenting wound assessments. This feature will be available on the web portal first and on the mobile application as part of a future release.
When a user selects UTD numbers will be removed from the depth field and users will not be able to add any numbers to the depth field while UTD is toggled.
If a user unselects UTD, depth will show 0 and allow a user to change them again.
When UTD is enabled the .depth phrase used in Phrase templates will resolve as UTD instead of numbers.
When UTD is enabled the .volume phrase used in phrase templates will resolve as UTD instead of numbers.
In any report that contains depth as a field, the report will show the result UTD instead of a number.
Note: There is a Practice Permission available to restrict UTD to only "Pressure Ulcer/Injury" etiology with a Stage/Severity of Unstageable or Deep Tissue Injury. Ask your DocNow Account Manager for more details.
